The Nature of “Disease X,” the Potential Source of the Next Pandemic

Disease X and Preparing for the Next Pandemic

As the COVID-19 pandemic gradually recedes, health authorities are turning their attention to an ominous possibility – Disease X, a term coined by the World Health Organization (WHO) to describe a potential pathogen capable of sparking the next global pandemic. Dame Kate Bingham, who chaired the UK’s Vaccine Taskforce, warns that this hypothetical threat could claim as many as 50 million lives, potentially dwarfing the impact of COVID-19.

Bingham emphasizes the urgency of this situation, suggesting that Disease X might already be in the making, and that COVID-19, while significant, may not represent the ultimate threat. In fact, she suggests that Disease X could be up to 20 times more deadly than the coronavirus. Bingham envisions a scenario where mass vaccination campaigns would need to be orchestrated swiftly to counteract a highly infectious pathogen with the lethality of Ebola (with a 67 percent fatality rate).

While scientists have identified 25 virus families containing thousands of individual viruses, countless others remain undiscovered, further complicating the quest to predict and prevent Disease X.

Understanding Disease X:

Dr. Neha Rastogi, a Consultant in infectious diseases at Fortis Memorial Research Institute, Gurugram, sheds light on the concept of Disease X. She explains that Disease X is likely to be caused by a pathogen, often zoonotic, possibly an RNA virus. It may emerge in an environment where the conditions are conducive to sustained transmission. These emerging or re-emerging pathogens, labeled as X, represent a serious and ongoing threat, demanding vigilant surveillance and monitoring.

Furthermore, there’s a concern that Disease X could be an engineered pandemic pathogen, stemming from accidental laboratory incidents or even deliberate acts of bioterrorism. Such an outbreak could pose a catastrophic global risk.

Preventing Disease X:

Dr. Rastogi underscores the importance of containment and mitigation strategies, which involve the development and implementation of international guidelines to control bioterrorism. Immediate measures, such as travel restrictions and robust airport screening, would be crucial to prevent the spread of pathogen X across borders.

Additionally, she emphasizes the need for global cooperation among leaders, scientists, epidemiologists, and infectious disease experts to investigate, control, and eliminate Disease X. Widespread testing, surveillance, and rigorous contact tracing would be essential tools in containing potential outbreaks.

Dr. Rastogi advocates for concentrated efforts to accelerate the availability of medical resources, including test kits, vaccines, and first aid, both before and during a pandemic. Research into preventive measures, particularly vaccine development, should be prioritized to mitigate the severe consequences of Disease X.

Finally, she highlights the importance of a One Health approach, which seeks to bridge institutional gaps and prioritize risk assessment and mitigation strategies for emerging and re-emerging pathogens. This holistic approach is seen as a crucial response to prevent global catastrophes caused by diseases like X.
